Description

The NHC Chicago AmeriCorps member and Diabetes Care Coordinator will provide:

(1) outreach and educational programming to increase awareness of resources available to assist participants to better manage their condition. The program will provide health screenings, educational workshops and tips for lifestyle changes onsite at St. Bernard Hospital’s Ambulatory Care Center.

(2) The program will develop a referral program between the primary care, hospital, and community setting for those who are prediabetic and for those with type 2 diabetes.

St. Bernard Hospital is located in the community of Englewood located on the south side of Chicago. Englewood like many underserved communities has high rates of chronic health conditions. For the past 12 years Diabetes has been identified as one of the top five (5) chronic health conditions in St. Bernard Hospital’s (SBH) service area. St. Bernard Hospital service area includes the predominantly African-American communities of Englewood, West Englewood, Washington Park, Burnside, Grand Boulevard, Greater Grand Crossing, Auburn-Gresham, Roseland, Chicago Lawn and South Shore, which are some of the most disadvantaged neighborhoods on Chicago’s South Side. In the 2021 Community Health Needs Assessment, the percentage of adults in Englewood diagnosed with diabetes in twice the city average. The Hospital will utilize the AmeriCorps member to continue the service started by our current AmeriCorps member. The member will continue the outreach and education of the participants on the signs, physical impact and life changes needed to prevent and manage the disease.
