Description
Serve with Pittsburgh Mercy Operation Safety Net.
Responsibilities:
• Providing education to clients and refer them to providers
• Attending trainings and/or community events
• Maintaining an inventory of supply materials for patients, clients, and/or organizations
• Providing outreach in the community to promote services
• ADDRESSING STRUCTURAL OPPRESSION
o Review and assess intake assessments for clients utilizing OSN services
o Members will be expected to determine gaps in the intake process
o Members will develop strategies and implement changes in the intake process and documents in order to address health disparities within the marginalized community
• MEDICAL OUTREACH
o Engaging with individuals to assist with connection to medical team • Rounding in streets/camps with medical team • Preparing medical supply bags prior to rounds Street Outreach
o Member will provide basic survival supplies such as tents, sleeping bags, food, hygiene supplies to unsheltered individuals
o Connect individuals to the Allegheny Link to begin housing process
o Assist with connection to care (medical, behavioral health, resources)
o Participate in weekly county meeting to discuss unsheltered individuals
o Participate in 2 weekly Operation Safety Net meetings
