Description

The St. Joseph/Candler Georgia Infirmary programs, SOURCE (Service Options Using Resources in Community Environments) and CCSP (Community Care Services Program), seek to positively impact the lives of older adults and younger individuals with disabilities through innovative programs of enhanced and traditional case management, home and community-based support services, and primary medical care. Through the coordination of these services, outlined by individualized care plans, St. Joseph/Candler helps their members maintain good health and functional independence and avoid potential institutionalized care. A volunteer is needed as a community connections coordinator, providing companionship to the members and aiding members in fostering social connections in the community.
