Position: Health Educator
Collaborates with: Chicago Public Schools, Mobile Dental and Vision Van Partners, Community Based Organizations, Providers, Other Erie Staff
General Summary: The Health Educator is responsible for coordinating and facilitating health education programming and initiatives for patients and students at Erie Johnson and Erie Westside Clinics. The Health Educator will work with providers, teachers, and administrators
Principal Duties and Responsibilities:
• Provide support to programs within the department
• Participate in and complete necessary trainings
• Evaluate department programming efforts
• Build relationships with school staff and administrators
• Ensure proper and accurate documentation of programmatic effort
• Other duties as assigned
• Nutrition/Physical Activity
o Complete child and adolescent nutrition education training
o Coordinate 1:1 nutrition/health coaching meetings with CPS students; Assist students in developing self-management goals
o Provide individualized health coaching for adults with support of the provider
o Provide group education for community-patient adults with focus on healthy lifestyles
o Research possible nutrition education programs to implement at Johnson/Westside, including but not limited to Cooking Matters
• Teen Pregnancy and Prevention (TPP) Program
o Implement Chicago Public School curriculum with other AmeriCorps members to students in schools and community organizations
o Participate in TPP program training
o Complete “teach backs” to ensure understanding of curriculum
o Schedule TPP classes with teachers and school administration
o Develop content, prepare for, and teach TPP program in schools and within the community
o Implement pre/post-test evaluation of curriculum
• School-Based Health Support Programs
o Vision Van: collaborate with CPS Vision Care Specialists to arrange for bi-annual schedule for students to receive vision services at school
o Dental Van: collaborate with Advocate Dental Van to establish visit schedule for students to receive dental services as school
o Promote and schedule students for service
o Manage day of operations to ensure patients are seen and providers are supported
• Adolescent Health Campaigns
o Develop monthly adolescent health topics to address current and relevant health issues
o Host monthly lunch promotional efforts to promote health center services and provide education to students
• Outreach
o Promote Erie services city wide at health fairs and other community-oriented events
o Assist patients with enrollment in eligible health coverage options
o Screen patients for social determinants of health and connect them to resources within the community

Each fellow is provided with opportunity to be matched with a professional mentor, a spiritual companion, and counseling resources. There are three service days and trainings for all fellows each service year as well as mentoring if wanted.

Qualifications and Required Skills:
• College coursework in health or human services field (Bachelor’s degree preferred)
• Good verbal and written communication
• Proficient computer knowledge, skills, and experience with Microsoft office

• Cultural competency in working with underserved populations
• The ability to modify lessons according to audience’s age/ cognitive abilities
• Organizational skills when tracking program data
• Ability to work effectively with Erie/ NHC member teams and colleagues
